Dementia Support Workers and Dementia Advisers working from GP Practices in Bristol
This service aims to provide carers and people living with dementia with a point of contact who can provide signposting, information and support throughout their journey with dementia. By providing this service from GP practices, it has given dementia a focus in primary care, raised awareness, increased the number of people on the dementia register and enabled people earlier on in their journey to seek help that can then prevent crisis at a later date. The service addresses feedback from carers and people with dementia saying that they want a named person throughout their journey to provide them with information and signposting.
