Treatment, care and support will include
- awareness of presenting symptoms or signs warranting further diagnostic assessment
- initial assessment
- diagnostic assessment
- post-diagnosis information, support and education for patient and carers
- prescribing, and medicines management
- health promotion and prevention
- active planning of future needs and wishes
- signposting to support agencies
- monitoring of disease progression; regular review as the patient’s presentation changes, including review of carer’s needs
- advance planning and care towards the end of life.
