This improvement plan template has been produced as a resource for Clinical Commissioning Groups and general practices, to assist with the design and delivery of local improvement plans to achieve improved outcomes for people living with dementia, and their carers/families in line with Everyone Counts: Planning for Patients 2013/14, and Domain 2 of the NHS Outcomes Framework 2013-14.
