The Dementia care in primary care toolkit is designed to support GPs and primary care teams to improve the screening, diagnosis and effective management of dementia.

It provides tools that will help you to calculate the prevalence and incidence of dementia in your locality, along with tools that will help you to screen for people with dementia. the toolkit also offers tips for better record keeping, and signposts to further education and training resources.
